 Each time the doctor pulls it out   
He reads what he last wrote about . . .   
Well, I have no idea at all.   
Before he walks it down the hall  
He mutters condescendingly,   
Then makes more notes I'll never see. 
  
Indeed, this  chart I've never spied,   
Yet every nurse can look inside –  
So secretive, the whole affair,   
With contents no one deigns to share   
Since "confidentiality"  
Extends, it seems, to only me.
